Register as a New Patient
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.
If you have mislaid your medical card you will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the health care team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. You should should bring a urine sample and all current medication to this appointment.
Medical treatment is available from the date of registration. Please contact reception for further information.
If you wish to pre-register click on the links below to open the forms. When you have completed all of the details, click on the "Submit" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
Practice Catchment Area
In 2012, all GP practices were asked to agree an outer practice boundary. Outer practice boundaries are an expansion of a GP's original catchment area. Whereas in the past you may have had to register with a new GP even after only moving a few streets away, these outer boundaries can be a way to stay registered with your old GP.
You’ll still have to speak with your GP first and, if appropriate, the GP may keep you on the register. For example, a GP may refuse to keep you on the register if you need frequent home visits but don’t live close to the GP practice. If the GP covers a large area and the travel distance to your home is too long to justify regular home visits, then the GP can suggest that you register with a practice closer to your home.
However, if the GP decides you can stay registered, then you will continue to receive the full range of services, including clinically necessary home visit.

Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
